About The Event

The MHC engages to understand the people and the communities. We look to understand how these communities are different and their similarities. We aspire to anticipate the changing needs and meet those needs. In addition to services, the Mobile Health Collaborative collects input from attendees and families. All families are registered in a secure case management system. This improves access to services and continuity of care. Families are asked about their perception of their thoughts on healthcare and their community both the strengths, resources and needs and gaps. This information is reviewed to understanding of the challenges and the best way to leverage resources and strengths within the communities. This report is then reviewed by MHC to improve services and disseminated to the community and community leaders. The goal is to initiate discussion between community leaders and neighborhoods to use current resources to address the most urgent needs.
